I really haven't managed a project I have failed to deliver on the basis of budget or time, but have had projects descoped. This is often not the failure of the PM in the process, as in my case, but the expectations of leadership in their desire to deliver a complete solution. Note: There are many measures of project failure and success, and the question should maybe have been more specific at defining what they were looking for with respect to a "failed" project.
