I gave a specific story and made sure to focus on what I did (DO NOT say that you bumped it up to a manager - all of your answers need to be what YOU did) I explained a scenario where a coworker made a mistake and was out of the office when things went awry, and I said that I stepped up to help and all of the things I did to address the problem. Finally I wrapped up the story by saying that it was a success
