I think Its because they wanted to know if you followed the change management process to fix the problem or issue, cause if you follow change management there are lots of decisions and approvals you might have to take in such a big company. You can not just go ahead and fix the problems in live production environments, this is what I think could have been wrong in your handling of that question.
