IT service transtion has five phases, begins with Initiation, planning, implementation, observation and finally hand over phase. Budget and governance team engage at the beginning then top level resources, L3 L4 are hiered and based on effort metrics we implement resources, the transitioning happens, Knowledge transfer takes place, later in observation phase the delivery team demonstrate their KTs and complete Run books, later at the Handover we sign documents and final agreed feedbacks and close the transition process.
