I spoke of the problem Situation. Then led the team defined it using a DIMAC approach. Based on the break down of the contributing causes, I made the case for the causes and reviewed it with the supervisors. The Tasks were identified and then the Approach to solving the problem. Next the Result of the improvement and any Reflections on what could be done better including improving the procedures. (STARR approach).
