Responsibility & roles social Workers work directly with patients and their families and need to have compassion. Social Workers should be patient, calm and able to manage stress. They may need to provide treatment to patients in lieu of medical professionals in the event of emergencies while also providing therapeutic support to patients and their families as needed. Social Workers may also have the following duties: • Evaluate patients (existing, new and prospective) based on their needs, limitations and desires • Address patient concerns and goals while maintaining constant communication with the patient as well as a case manager • Build rapport with patients and their families • Coordinate care with PT and OT departments • Develop care plans for patients Responsibilities and Duties: • Assist the Community Work Centre – Head in implementing various activities of the CWC • Act as a link between community and school through regular home visits and identify dropouts • Conduct admission process of children and maintain the relevant documents • Prepare home visit reports, detailed case studies of children, CWC reports and minutes of the meeting • Organize training programmes for parents on topics related to education, health, and vulnerability. • Ensure overall attendance of children is increasing in line with the goals set by the organization. • Actively participate in Parents meeting and provide feedback on the progress of the child to the parent • Ensure Learning Disability (LD) certificates of all children are collected from the hospital • Coordinate and network with other civil society groups working in the social sector
