A 30 minute chat with CEO and MD. A bit unprofessional in parts because they asked about people I currently worked with and spoke disparagingly. I should have concluded then and there that this wasn't a good/professional management to work for.

First stage interview with team manager, second interview with MD. Both friendly and professional, happy to answer any questions you put to them. After two successful interviews, I was invited to come in for a trial with the team to see if it's a good match - a two way street.
