Eliot Community Human Services' Care Coordination Division is proud to announce significant achievements in its Behavioral Health Community Partner (BH CP) and Health Home programs. The BH CP program has surpassed expectations, with over 70% of Health Related Social Needs screenings identifying critical areas such as housing and food security. Care Managers have efficiently completed Person-Centered Care Plans 22 days faster than state benchmarks. The Health Home program has also demonstrated excellence, achieving a 65% rate of member visits within 48 hours of Transitions of Care, exceeding the 60% benchmark. These accomplishments underscore Eliot's commitment to delivering impactful, person-centered care and driving excellence in community services.
