Complex conditions, including cardiometabolic conditions, require more support than quick, and sometimes sparse, appointments with your provider. It requires support during the time between visits. When evaluating virtual care, it’s important to assess if the program allows for equitable access across all regions, even in locations with limited in-person service availability. In the case of Omada’s program: ⭐ 45% of Omada messages are received outside of normal business hours. ⭐ Over 3.6 million messages were sent to members by the care team in a single year. ⭐ 58% of messages are initiated by a health coach - with the care team conducting proactive outreach to better understand members’ experiences and build rapport. Did you know certain racial/ethnic groups may be more likely to have comorbid mental health and cardiometabolic conditions? (see here: https://lnkd.in/eu4ZscmF) In a recent analysis, Omada’s clinical research team assessed changes in depression and anxiety symptoms among members in our prevention, hypertension and diabetes programs. One in eight U.S. adults say they have tried a GLP-1. But are they getting the support they need for long-term success? Omada's Enhanced GLP-1 Care Track offers care through all stages of the GLP-1 journey. With the personalized, compassionate support of our care teams, 82% of Omada members taking GLP-1s see amplified weight loss. Even four months after discontinuing the medication, Omada members maintained their weight loss (-0.1% weight change on average).
