In person interview with the hiring manager, one on one. It was very laid back and conversational. This was for a contracting position, there was another interview several months later that was structured.

First round was a basic screening with an HR rep. I was invited to an on-site round with the Hiring Manager, another manager, and the lead. I don't remember the questions, but I remember that they were not hard. The interview was relatively easy. I received an offer, accepted, and worked there for about 2 years.
